1 新员工入场流程New Employee Entry Process

👤 入场步骤Entry Steps
  1. 新员工参加政府要求的 苏安码培训 和 体检New employees must complete the government-mandated Su'an Code training and undergo a medical examination.
  2. 完成 三级教育，签署 劳务合同Complete the three-level safety education and sign the labor contract.
  3. 提交培训申请 — 将新员工资料提交到 Primelayer 系统Submit the training application by uploading new employee data to the Primelayer system.
  4. 新员工在保安室登记并阅读 安全须知Register at the security office and read the Safety Instructions.
  5. 授权人到门岗接待新员工，并在登记表上 确认签名An authorized person shall receive the new employee at the gate and sign the registration form to confirm.
  6. 新员工参加 入场培训 并 考试合格Attend the site induction training and pass the examination.
  7. 考试合格人员到保安室录 人脸识别系统，并由制卡员 制卡Upon passing, enroll in the facial recognition system at the security office and have the ID card issued.
  8. 承包商人员将员工信息录入 实名制考勤系统 *Contractor staff must enter employee information into the Real-Name Attendance System.* * ⚡ 业主的设备类包商，可不录入实名制考勤系统 ⚡ Equipment subcontractors (owner-procured) may skip the attendance system.
  9. 新员工佩戴保安发放的访客证进入培训室New employee wears visitor badge (issued by security) to enter training room.
📌 重要条件📌 Key Requirements
  • • 年龄 18–60 周岁• Age: 18–60 years.
  • • 每人每次事故赔付额不低于 100 万 的意外保险或雇主责任险（建筑施工类）• Must hold accident insurance or employer’s liability insurance (construction) with a minimum coverage of RMB 1 million per incident.
  • • 三级教育时长满足政府要求（不低于 50 学时）• The duration of the three-level education must meet government requirements (≥ 50 hours).

  • 劳务合同Labor contract
  • 三级教育卡（不低于 50 学时）Three-level education card (≥ 50 hours)
  • 苏安码是 绿码Su'an Code status must be Green.
  • 特种作业人员：提交 特种作业操作证（住建部颁发） Special Operations Personnel: Submit the Special Operation Certificate (issued by the Ministry of Housing and Urban-Rural Development). 压力容器管道焊接：需增加 市场监督管理局焊工证 Pressure Vessel/Pipe Welding: Additionally require the Welder Certificate issued by the Market Supervision Administration.

2 访客入场流程Visitor Entry Process

🚪 访客步骤Visitor Steps
  1. 访客到现场，联系 被访人Upon arrival, the visitor must contact the host.
  2. 被访人向 项目授权人 申请访客进入要求The host must request visitor entry approval from the Project Authorized Person.
  3. 访客在保安室 登记 并阅读 安全须知The visitor must register at the security office and read the Safety Instructions.
  4. 被访人到门岗 接待访客，并 签名The host must receive the visitor at the gate and sign the registration log.
  5. 保安登记后 发放访客证Security issues visitor badge after registration.
  6. 访客佩戴访客证 进入现场The visitor must wear the Visitor Badge to enter the site.
  7. 被访人 全程陪同 访客The host must accompany the visitor at all times.
  8. 访问结束，由被访人陪同离开现场并将访客证 归还至门卫Upon completion, the host must escort the visitor off-site and return the badge to the gate guard.

  • • 如果是 施工人员，则按照 新员工入场流程• If construction worker, follow New Employee Entry Process.
  • • 项目授权人员名单详见现场公告• Authorized personnel list is posted on site.

3 施工工具 & 设备入场Tools & Equipment Entry

🔧 入场步骤Entry Steps
  1. 承包商人员在 Primelayer 系统 提交设备资料Contractor personnel must submit equipment details via the Primelayer system.
  2. 承包商施工人员在 Primelayer 系统 提交 设备材料进场申请表Submit the Equipment/Material Entry Application via the Primelayer system.
  3. C3 安全人员 检查工具、设备，合格后贴 检查合格标签C3 Safety personnel must inspect tools/equipment and affix an Inspection Pass Label upon approval.
  4. 保安核对 入场申请单 和 检查合格标签Security must verify the Entry Application Form against the Inspection Pass Label.
  5. 承包商将工具、设备 进入现场Contractor brings tools/equipment onto site.
📌 关键要求📌 Key Requirements
  • • 所有进入现场的 施工器具 都需提交材料设备进场申请单并获批准• All construction tools entering the site require a submitted and approved Material/Equipment Entry Application.
  • • 施工器具 / 特种车辆入场前必须经过 C3 安全部检查，合格才能入场• Tools and special vehicles must pass the C3 Safety Department inspection before entry is permitted.
  • • 南门 为物流通道• The South Gate is designated as the logistics channel.
  • • 车辆进出现场都要有 车辆引导员 引导• All vehicles entering or exiting must be guided by a Vehicle Marshal.

4 材料入场流程Material Entry Process

📦 入场步骤Entry Steps
  1. 施工材料及设备到达 现场大门Construction materials and equipment arrive at the site gate.
  2. 承包商施工人员在 Primelayer 系统 提交 设备材料进场申请表Submit the Equipment/Material Entry Application via the Primelayer system.
  3. 施工单位负责人 审批The Contractor Site Manager must approve the application.
  4. C3 授权施工人员 检查材料，并 批准 材料入场申请C3 authorized construction staff must inspect the materials and approve the Material Entry Application.
  5. 保安核对无误后 放行 进入现场Security verifies and releases for entry.
📌 关键要求📌 Key Requirements
  • • 所有进入现场的材料都需提交 材料设备进场申请单 并获批准• All materials entering site require a submitted and approved Material/Equipment Entry Application.
  • • 南门 为物流通道• The South Gate is the logistics channel.
  • • 三轮车、摩托车、面包车 和 混装化学品的车辆 不得入内• Tricycles, motorcycles, vans, and vehicles carrying mixed chemicals are prohibited from entering.
  • • 车辆进出现场都要有 车辆引导员 引导• All vehicles entering/exiting must be guided by a Vehicle Marshal.

5 材料 & 设备出场Material & Equipment Exit

🚚 出场步骤Exit Steps
  1. 施工材料及设备需要出场Materials and equipment need to leave site.
  2. 承包商施工人员在 Primelayer 系统 提交 材料、设备出场申请Contractor personnel must submit the Material/Equipment Exit Application via Primelayer.
  3. 施工单位负责人 在 Primelayer 系统审批The Contractor Site Manager must approve it in Primelayer.
  4. C3 授权施工人员和安全人员 签署材料出场申请C3 authorized construction and safety personnel must sign the Exit Application.
  5. 保安核对出场的材料和申请所列物品单是否 一致Security must verify that the outgoing materials match the items listed in the application.
  6. 保安核对无误后 放行，材料设备由申请人带出现场Security confirms and releases; materials/equipment taken out by applicant.
⚠️ 严厉管控⚠️ Strict Control
  • • 所有的材料设备离场必须有 C3 批准的出门申请单• All materials/equipment leaving site must have a C3-approved exit application.
  • • 无批准的出门单，任何材料设备离场将按照 偷窃 处理• Any removal of materials/equipment without an approved exit permit will be treated as theft.
  • • 车辆进出场，保安有权 检查驾驶室和车厢 并拍照• Security reserves the right to inspect the cab and cargo area and take photographs.
  • • 车辆进出现场都要有 车辆引导员 引导• All vehicles entering/exiting must be guided by a Vehicle Marshal.

6 施工作业票审批流程Work Permit Approval Process

📋 审批步骤Approval Steps
  1. 作业负责人在 Primelayer 系统 提交 作业申请The Work Supervisor must submit the work application via the Primelayer system.
  2. 承包商施工负责人 和 现场安全管理人员 确认并审批The Contractor Site Manager and Site Safety Manager must review and approve.
  3. 承包商施工负责人与 C3 施工负责人、安全管理人员 进行充分沟通The Contractor Site Manager fully communicates with C3 Construction and Safety managers.
  4. C3 施工人员和安全 审批并 预签署作业票C3 Construction and Safety personnel must approve and pre-sign the Work Permit.
  5. 承包商安全人员 检查现场施工条件，合格后 签署作业票Contractor Safety personnel must inspect site conditions and sign the permit upon compliance.
  6. C3 安全人员 核实现场施工条件，合格 最终批准 作业票后生效C3 Safety personnel must verify site conditions; the permit becomes effective only after final approval.
  7. 作业结束后承包商 关闭作业证The contractor must close out the permit upon completion of work.
⏱️ 时效性
一般作业许可证有效期 7 天；专项作业许可证有效期 1 天。
签署时间：每天下午施工与安全协调会后签 第二天 的作业许可。C3 安全工程师会在现场签署当天的许可证。 ⏱️ Validity
General Work Permits are valid for 7 days; Special Work Permits are valid for 1 day.
Signing time: Permits for the following day are signed after the daily Construction & Safety Coordination Meeting. C3 safety engineers sign same-day permits on site.

⚠️ 重要前置条件⚠️ Prerequisites
  • • 申请作业票之前必须要有批准的 施工方案 和 安全风险分析（JSA）。JSA 模版单独发送。• An approved Construction Plan and Job Safety Analysis (JSA) are required prior to permit application. JSA template provided separately.
  • • 每项作业必须有批准的作业票。未获得 C3 安全人员批准 之前不得施工。• Every work activity requires an approved permit. No work without C3 safety approval.
  • • 作业票默认时间点为单天 下午 7 点（吊装为 6 点），如需延时需提交 延时作业申请，批准后可作业。• The default permit start time is 7:00 PM (6:00 PM for lifting operations). For extension, submit an extension request for approval.

7 施工方案 & JSA 审批Construction Plan & JSA Approval

📄 审批步骤Approval Steps
  1. 包商技术负责人在 Primelayer 系统 提交 施工方案The Subcontractor Technical Lead must submit the Construction Plan via Primelayer.
  2. C3 专业负责人和安全人员 在 Primelayer 审核C3 Discipline Lead and Safety personnel must review it in Primelayer.
  3. 包商技术负责人依据 C3 审核意见 修改，完成最终版The Subcontractor Technical Lead revises based on C3 comments and finalizes.
  4. C3 专业人员负责和安全人员 批准C3 Discipline Lead and Safety personnel approve.
  5. C3 管理包主和技术负责人 批准C3 Package Manager and Technical Lead approve.
  6. 承包商将在 Primelayer 批准的方案和 JSA 上传到 ACC 系统The contractor must upload the Primelayer-approved Plan and JSA to the ACC System.
  7. C3 相关人员 和 业主相关人员 在 ACC 系统批准C3 relevant personnel and owner representatives approve in ACC.
  8. ✅ 最终获得批准✅ Final approval obtained.

  • • JSA 模版 单独发送• JSA template provided separately.
  • • 施工方案必须上传 ACC 系统 获得最终批准• The Construction Plan must be uploaded to the ACC System to obtain final approval.

8 违规处理流程Violation Handling Process

🚫 处理步骤Handling Steps
  1. 发现违规Violation detected.
  2. 收回 违规人员胸卡（由包商安全人员负责）Confiscate the violator’s ID badge (responsibility of Subcontractor Safety Personnel).
  3. 违规人员 清出施工现场The violator must be removed from the construction site.
  4. 根据违规情况处理（重新培训 或 开除）Action based on severity (retraining or dismissal).
  5. 违规人员参加 重新培训Violator undergoes retraining.
  6. 胸卡 剪角 后发回违规人员The badge will be corner-cut and returned to the violator.
  7. 工人 回到现场作业Worker returns to site.
  8. 工人被开除，开罚单If dismissed, fine issued.
⚠️ 严厉措施⚠️ Strict Measures
  • • 收回违规人员胸卡由 包商安全人员 负责• ID badge confiscation is done by Subcontractor Safety Personnel.
  • • 罚单依据合同的 罚款条例 对承包商处罚• Fine is imposed on contractor per contract penalty clauses.
  • • 违规人员的胸卡将被 剪角，并记录• Badge will be corner-cut and recorded.
  • • 剪角三次 将被开除出现场• Three corner-cuts will result in permanent dismissal from the site.

9 隐患整改流程Hazard Rectification Process

🔍 整改步骤Rectification Steps
  1. 现场发现 安全隐患（不符合项）Identify site safety hazards (non-conformances).
  2. 在 Primelayer 系统 填报Report the hazard in the Primelayer system.
  3. 整改责任人 制定整改措施及安排整改The Corrective Action Owner must define measures and arrange for rectification.
  4. 现场整改完成，反馈 到 Primelayer 系统Upon completion, enter the rectification feedback into the Primelayer system.
  5. 现场 复查 整改情况On‑site re‑inspection of rectification.
  6. 确认完成后 关闭整改After confirmation, close out the rectification.
📌 关键要求📌 Key Requirements
  • • 隐患必须按照计划的 期限完成 并反馈• Hazards must be rectified within the planned deadline and feedback provided.
  • • 现场的文明施工，必须 工完场清，当天清理干净现场；否则将 拒签 第二天的施工作业票• Maintain site housekeeping: adhere to "clean-as-you-go" principles and clear the site daily. Failure to do so will result in the refusal of the next day's Work Permit.

10 事故调查流程Accident Investigation Process

🚨 调查步骤Investigation Steps
  1. 现场 及时上报 事故情况Report the incident promptly from the site.
  2. 承包商现场负责人立即安排人员 救援及就医The Contractor Site Manager must immediately arrange for rescue and medical treatment.
  3. 事故处理结束后，承包商上报 书面事故情况说明After initial handling, the contractor must submit a written incident report.
  4. C3 安全经理（或授权人） 组织事故调查并确定 事故整改措施The C3 Safety Manager (or delegate) must lead the investigation and define corrective actions.
  5. 跟踪 事故整改措施落实情况Track implementation of corrective actions.
  6. 组织 专项事故教育培训Conduct special incident education and training.
  7. 整理汇总上传到Primelayer系统记录Compile and upload the records to the Primelayer system.

  • • 所有事故 将被调查，并采取有效的预防措施，避免再次发生• All incidents will be investigated, and effective preventive measures will be implemented to prevent recurrence.
